Financial Policy

All fees, including co-payments and co-insurance payments, are due at the time of service. Payment is accepted in the form of cash or credit card.

Insurance Policy

  • Medicare: Dr. Buttar is a participating provider of Medicare.
  • Private insurance: We participate with most insurance plans. Please call the office to see if we will accept your particular plan. If we do not participate with your plan, then payment is expected at the time of your visit. We will gladly submit a claim to your insurance company on your behalf or we can give you the appropriate forms to submit to your insurance company for reimbursement.
  • No-Fault: We do not accept No-Fault insurance. You may still see Dr. Buttar for your injuries however payment is expected up front. We will gladly provide you with the paperwork to submit to your No-Fault insurance company for reimbursement.
  • Self-Pay: Initial consultation fee is $200. Follow-up visits are $100+. The cost for injection procedures vary depending on the type of service.

Cancellation Policy

If you miss your appointment or cancel your appointment without advanced notice, it may prevent Dr. Buttar from helping other patients in need. In consideration to other patients and to our practice, kindly give 24 hours notice for a follow-up and 48 hours notice for a procedure or initial consultation if you are unable to keep your appointment. Noncompliance with this policy will result in a late-cancellation fee before scheduling another appointment.
